Each nomination must include:
• Nominee’s full name, title, organization, and contact information
• A professional biography (250–500 words)
• A nomination statement (500 words maximum) detailing the nominee’s scope of leadership, measurable impact, and alignment with the award’s criteria
• At least one letter of recommendation (optional but encouraged)
• A high-resolution professional headshot
The honoree will be formally recognized at the Unleash Your Inner Worth Annual Gala.
